All required documents must be thoroughly completed, signed, and submitted to Arya prior to your first appointment. It is crucial that we have accurate and up-to-date information to provide the best possible care for your pet(s). Without this completed form, we regret that we will be unable to proceed with any treatments or services for your pet(s).

Kindly note that we require a deposit for all new clients/patients’ first appointments. This is a non- refundable deposit that will be credited to your pet’s first visit.
The cost of the new patient/new client consultation is $350 and includes a complete nose-to-tail physical exam, diet consultation, medication/supplement review and overall integrative medicine consultation: including but not limited to Chinese medicine evaluation. Arya Animal Acupuncture is a Specialty Integrative Practice lead by a team doing revolutionary treatments not yet practiced widely. The level of expertise time and care lead to us creating what we know is value to you and your pets.

We are a specialty integrative care veterinary practice that practices an open concept where pets are able to remain in the presence of their owners for many of the different treatments and procedures offered. This open concept may not be a fit for every client or their pet.
